Educational Guardianship (including Parent Affidavit)

Enrollment takes place at the school with the school registrar. If you need an Educational Guardianship or Parent Affidavit as part of your documentation to take to the school, see below.

When it is necessary for a student to live with someone other than his/her parents, or when the parent/guardian(s) don't live at the same address, the individual with whom the student is living must have one of the following documents in order to enroll at their school. 
  • Court Custody Documentation
    • If there is a Department of Children and Families Placement Letter (signed by a judge); an Every Student Succeeds Act (ESSA) staffing is also requiredContact Student Advocacy - Natasha Taylor
  • Educational Guardianship or Parent Affidavit from Office of Student Enrollment*

Please note: Please follow the instructions below for applications - Student Enrollment does not accept applications via our email address.

NOTICE: Families are seen by appointment only at the Office of Student Enrollment.

Parent/Guardian lives IN Orange, Brevard, Lake, Osceola, Polk, Seminole or Volusia counties:

If you are seeking educational guardianship, you will need:
  • Guardian's Driver's License/State ID
  • Guardian's Proof of Residency, as stated under Verification of Residence 
  • Copy of the child's birth certificate
  • Notarized statement from custodial parent and copy of ID
  • Custodial parent's Proof of Residency

AND at least one of the following:

  • If the parent is deceased: copy of the obituary or death certificate, showing date of death
  • If the parent is hospitalized: letter from the doctor on letterhead stating when the parent was admitted and possible release date
  • If the parent is incarcerated: proof of incarceration
  • If the parent is deployed: military deployment orders

If none of the above situations apply, the person seeking educational guardianship may need to apply for custody by an extended family member in court before coming to Student Enrollment.

Note: More Documentation may be required depending on circumstances on a case by case situation.

Parent/Guardian lives OUTSIDE of Florida or OUTSIDE Orange, Brevard, Lake, Osceola, Polk, Seminole or Volusia counties:

If you are seeking educational guardianship, you will need:

  • Guardian's Driver's License/State ID
  • Guardian's Proof of Residency, as stated under Verification of Residence
  • Copy of the child's birth certificate
  • Notarized statement from custodial parent and copy of ID
  • Proof of custodial parent's residency outside of Florida or Orange, Brevard, Lake, Osceola, Polk, Seminole or Volusia counties

Make an appointment for Educational Guardianship

Step 1 - Download an Educational Guardianship Application and save to your computer. Do not fill out in Chrome, use Adobe Reader only.

Step 2 - Complete, save, print the form to bring to the appointment. (our office will provide the form at the appointment if you cannot bring one)

Step 3 - Gather all required documentation listed above.

Parents/Guardians NOT residing at the same address (Parent's Affidavit)

If you are seeking a Parent's Affidavit, you will need:

  • Both Parent's/Guardian's Driver's License/State ID with current addresses
  • Both Parent's/Guardian's Proof of Residency, as stated under Verification of Residence
  • Copy of the child's birth certificate
  • Court documentation (Divorce decree, parenting plan)
  • If separated, but still married, proof of separation from marital home (i.e., homestead removal).

Step 1 - Download a Parent's Affidavit Form and save to your computer. Do not fill out in Chrome, use Adobe Reader only..

Step 2 - Complete, save, and print the form. You may  get the form notarized or we will notarize the form in our office, both parents must attend if both signatures are to be notarized (form provided in office if necessary)

Step 3 - Gather all required documentation listed above.
